De nuevo con vosotros y esta vez diría que con un post esporádico y que también , aparte de pretender ayudar (como siempre), creo que invita a la reflexión. Es otro alto en el camino, como muchos de los que he redactado y publicado en mis últimos 59 posts desde el 16 de agosto de 2016, fecha en la que nació este blog (el primer aniversario ya está muy cerca y con el permiso de todos vosotros lo celebraré con un post «homenaje»).
Y es un alto en el camino para reflexionar sobre los impactos de una enfermedad tremendamente compleja, que produce una incertidumbre vital que, a pesar de lo que algunos puedan pensar, es muy difícil de gestionar. Supone estar siempre alerta, no bajar nunca la guardia, y el desgaste que eso produce es enorme.
Un lector de este blog comentó hace poco que se siente solo, aún y estando supervisado por un equipo médico excelente y de primer nivel. Es algo que considero importante para remarcar y para profundizar, un sentimiento que muchas veces no nos atrevemos a exteriorizar y comentar, pero que nos invade en muchas ocasiones y que no sabemos como gestionar.
Opino que nos sentimos solos por múltiples razones; la más importante quizás es consecuencia de que padecemos una enfermedad muy compleja, aún muy desconocida por parte de los propios especialistas y que nos cansamos de escuchar que afecta a cada persona de un modo distinto, con lo que es muy difícil (a veces imposible) poder compartir los síntomas y los efectos que uno tiene con otros pacientes e incluso con los propios especialistas.
Y es cierto, esta semana tuve el placer de reunirme con otros enfermos y considero que constatamos o vimos lo de casi siempre: problemas parecidos pero nunca un mismo cuadro clínico, compartimos los síntomas que cada uno tenemos y compartimos hechos como podrían ser el que uno tiembla pero no tiene rigidez, otro (yo mismo) tiene rigidez y bloqueos en la marcha que los otros no tienen, otra tiene inestabilidad, mareos e incluso visión doble ; hablamos y hablamos, medicación distinta, efectos secundarios distintos, cuadros clínicos diferentes (uno tiene problemas de colesterol y/o de glucosa, otros no, etc). Pero amigos, lo cierto es que sí que encontramos muchas cosas en común:
casi todos coincidimos en que nuestro estado físico y mental (muy importante) está sometido a fluctuaciones constantes, hay ratos buenos y ratos malos ( y ratos horribles) y, en ocasiones, podemos entender los motivos pero muchas otras (quizás la mayoría) no los entendemos. Es difícil convivir con la fluctuación dado que genera inestabilidad, desequilibrio, dificultad para planificar; genera problemas recurrentes ( y ello desgasta mucho) en la esfera personal, en la relación con la familia, en el ámbito social. Requiere tener una fortaleza mental diferencial.
O estamos incapacitados laboralmente o de baja médica o los que trabajan lo hacen con jornada reducida. Cada uno debe hacer lo que deba y pueda ; es imposible extrapolar o aconsejar dado que si cada paciente es único, lo mismo ocurre con el puesto de trabajo y empresa.
Dormimos menos de lo normal y en muchas ocasiones mal. Es muy importante descansar porque nos permite estar en las mejores condiciones físicas y mentales para «lidiar» con los síntomas. Historias acerca de nuestros períodos nocturnos de insomnio en los que hacemos lo que podemos: aprovechamos para “crear” (escribir, definir proyectos, hacer manualidades), nos agobiamos, nos aburrimos, nos entristecemos, despertamos al cuidador, a la pareja en busca de alguien con quien hablar y compartir estos momentos, o en busca de alguien que nos pueda masajear una musculatura rígida y contraída, o nos pueda ayudar a levantar e ir al baño, o infinidad de cosas que uno puede plantearse hacer en esos períodos nocturnos en los que (para los demás) impera la calma.
Aparte del movimiento y la marcha, quien más quien menos tenemos problemas al hablar porque vocalizamos mal, nuestro tono es más bien bajo y algo monótono, y lo peor, empezamos a tener indicios de sufrir la temida disfagia, las dificultades para tragar adecuadamente los alimentos. Esas dificultades pueden derivar en una aspiración, en que el alimento entre en los pulmones y no vaya por el esófago, y eso puede representar un riesgo muy serio de infección y de complicaciones graves (neumonía). De esto se deriva la importancia de visitarse con un especialista (logopeda en este caso) para ver si existe o no disfagia y en caso de existir, poder recibir unas pautas adecuadas para poder eliminar o reducir significativamente el riesgo de aspiración.
Tenemos miedos, ansiedad, alguno depresión, por la gestión del día a día, de la familia, de lo limitaciones vendrán, quien nos ayudará y cuidará cuando lo necesitemos. Lo llevamos lo mejor que podemos pero es imposible ser inmune a ello porque todos somos humanos y estamos hablando de efectos que, algunos de ellos, ponen o pueden poner en peligro a corto plazo, nuestra supervivencia. Aquí tenemos de todo; ejercicio físico, ayuda psicológica o psiquiátrica, autoayuda (técnicas de relajación, de gestión de la ansiedad y el pánico), meditación, yoga, mindfulness, seguimiento de maestros espirituales, medicina alternativa, etc.
Solemos tomar medicación parecida pero distinta, distinta en el medicamento específico o distinta en la dosis. Es muy difícil en ocasiones saber los motivos por los que un paciente toma A y otro toma B, debido que depende de la historia clínica de cada uno, del momento y edad del diagnóstico, del neurólogo (puesto también ocurre que “cada maestrillo tiene su librillo”), de la aceptación de los medicamentos por parte de cada uno, del “tipo” de Parkinson (si temblor o rigidez, si avanza rápido o no, etc).
Vemos que los especialistas cada día saben más acerca de esta enfermedad pero seguimos estando sujetos (y va para largo….) al “prueba y error“, a los otras veces comentados ajustes de medicación (que casi siempre producen nuevos efectos secundarios o colaterales..) en búsqueda de una calidad de vida aceptable paliando los efectos de los nuevos síntomas, de los zarpazos del “monstruo”. Aquí me siento afortunado de que existan distintos medicamentos (y cada vez más) para poder “gestionar” razonablemente tanto los síntomas motores como los no motores y es algo que siempre hemos de poner en valor y no olvidarnos. Pero soy consciente de lo que una vez me dijo un persona con gran experiencia en nuestra sanidad y es que no somos enfermos terminales pero estamos y estaremos (posiblemente la mayor parte de nosotros) recibiendo cuidados paliativos de forma crónica. Aquí, me revelo, e intento ser paciente activo y en la medida de lo posible ayudar a la investigación y a la ciencia para que todo vaya un poco más rápido en busca del objetivo final (cura de la enfermedad). Os hablaré de ello y de proyecto/s concretos en próximos posts.
Coincidimos en que cuando tenemos cualquier problema de salud es difícil, y a veces muy difícil, concluir si es o no algo derivado u ocasionado por la enfermedad de Parkinson. Y es difícil para los especialistas, pues imaginaros para la medicina general….no quiero ni pensar en que un día me tengan que ingresar por cualquier motivo…..teniendo Parkinson…..entonces sí que seré un paciente activo, experto…..por instinto de supervivencia, para asegurarme de algo tan sencillo como es el seguir tomando las pastillas adecuadas y para asegurarme de que no hay contraindicaciones o interacciones con la medicación que el ingreso hospitalario pudiera requerir durante el mismo.
Y amigos, coincidimos en lo más importante, en que hemos de ser fuertes, tener siempre una actitud positiva, en que nos hemos de reunir y hablar y compartir mucho más a menudo, en que nos tenemos de ayudar, en que nunca hay que rendirse, nunca. Ya sabéis lo que dice aquella canción del jefe…..
En general coincidimos en que el diagnóstico nos cambió la vida para siempre, convirtiéndonos en otras personas y ahí es donde está la oportunidad o la solución: no hace falta ser valiente y decidir hacer nuevas cosas que uno lleva dentro desde siempre y no hace falta porque la enfermedad ha decidido por nosotros, nos ha cambiado la vida de forma forzada. Como decía Francesc, mi psicólogo, en un escrito inolvidable publicado en un post antiguo en este blog: bienvenido a ti mismo !!
Y aquí está la gestión, tanto de la complejidad de la enfermedad como de la incertidumbre vital: vivir el momento, cada segundo, haciendo lo que más nos guste en la medida en que física y mentalmente lo podamos hacer. Y no hemos de dar explicaciones ni auto justificarnos de nada; hemos de hacerlo y ya está….se trata de convertir un problema o una amenaza en una oportunidad, y no darle excesivas vueltas.
Es bastante obvio todo lo que acabo de escribir pero vale la pena no perderlo de vista. Tenemos una incertidumbre vital, algo más fundada de la que tiene cualquier persona sana, pero tenemos la certeza de que nadie mejor que nosotros tiene pista libre para hacer y ser lo que más lo guste.
El vaso, medio lleno siempre, aunque tengamos calambres, caídas, bloqueos constantes de la marcha, tristeza repentina, apatía, durmamos mal o peor, dificultad para hablar, para tragar la comida, para vestirnos, para asearnos. Aunque a veces estemos muy tristes, solos, lloremos, tengamos ansiedad y miedo, no hay opción, únicamente sobreponerse y decirnos aquello que una buena amiga siempre se dice a si misma…..de momento……me pasa esto o aquello “de momento”, y es de momento porque esto fluctúa y no siempre acecha el monstruo, hay ratos, muchos ratos buenos y hay que quedarse y centrarse en eso, en que después de un mal momento vendrá un buen momento….
Y el título de este post lo traduciría como tormenta, que es como definiría lo que me está ocurriendo últimamente. Si pienso sólo en los síntomas motores y no motores que tenía un año atrás, mi evolución es preocupante (aunque clínicamente pudiera serxplicable). Hace un año, no tenía bloqueos o congelaciones de la marcha, no me cansaba físicamente ni la mitad que ahora, dormía mucho mejor, no tenía ningún problema en cuanto a la voz ni en cuanto a la deglución, me movía casi con total normalidad a la hora de darme la vuelta en la cama, estaba menos triste y preocupado (lógicamente). Ahora, en este momento, estoy más medicado, mis fases normales (fases “on”) pueden sumar 6 o 7 horas al día, siendo el resto fases intermedias o fases “off” (no puedo hacer nada, ni conducir, ni ir a comprar, ni jugar con mis hijos, ni prácticamente moverme). En parte pienso que es producto de la estación del año (el calor me consume y lo regulo francamente mal), a haber tomado antibióticos por otro tema, a estrés y ansiedad producidos por la propia situación que retroalimenta dicha sensación.
En mi última conversación con mi neuróloga me dijo que su conclusión es que, hoy por hoy, estoy “moderadamente fotut” (traducido sería moderadamente mal) y eso es muy subjetivo dado incluye un juicio de valor, lo moderado para uno puede ser más grave o menos grave que para otros….. Pero me quedo con lo que “de momento” estoy así y creo y confío en aquello de que «después de la tormenta viene la calma»».
Y si no, da igual, observo un momento a mi hijo de 15 años o a mi hija de 6 años, y apreto con fuerza mis puños para reanudar la marcha, para salir del bloqueo y continuar hacia adelante. GO !!
Parece que lo de apretar los puños ayuda debido a que después de una contracción muscular viene una expansión y es puede ser el motivo por el que puedo mover uno de mis dos pies cuando hago ese ejercicio. Un pequeño truco para cerrar este post con alegría, como siempre.

Again with you and this time I would say with a sporadic post and also perhaps, apart from pretending to help (as always), invites reflection. It is another stop on the road, like many of those I have written and published in my last 59 posts since August 16th, 2016, date on which this blog was born (the first anniversary is already very close and with the permission of all I’ll celebrate with a dedicated post or a post tribute).
And it is a high point on the way to reflect on the impacts of a tremendously complex disease, which produces a vital uncertainty that, despite what some may think, is very difficult to manage. It means being always alert, never lowering the guard, and the wear that it produces is enormous.
A reader of this blog recently commented that he feels alone, still and being supervised by an excellent medical team and first level. It is something that I consider important to emphasize and to deepen, a feeling that many times we do not dare to externalize and comment, but that invades us many times and we do not know how to manage.
I think we feel alone for many reasons; the most important is perhaps because we suffer from a very complex disease, still very unknown by the specialists themselves and we get tired of hearing that affects each person in a different way, making it very difficult (sometimes impossible) to be able to share the symptoms and effects that one has with other patients and even with the specialists themselves.
And it is true, this week I had the pleasure of meeting with other patients and I consider that we have seen or seen what we almost always have: similar problems but never the same clinical picture, we share the symptoms that each one has and we share the one that shakes but does not have stiffness, other (myself) has rigidity and blockages in the march that the others do not have, another has instability, dizziness and double vision; we talk and talk, different medication, different side effects, different clinical pictures (one has cholesterol and / or glucose problems, others do not, etc.). But friends, the fact is that we do find many things in common:
We all agree that our physical and mental state (very important) is subject to constant fluctuations, there are good times and bad times (and horrible times) and sometimes we can understand the reasons but many others (perhaps most) do not understand them . It is difficult to coexist with the fluctuation since it generates instability, imbalance, difficulty to plan; generates recurring problems (and this is very tiring) in the personal sphere, in the relationship with the family, in the social sphere.
Or we are incapacitated labor or of medical leave or those that work do it with reduced day. Each one must do what he must and can; it is impossible to extrapolate or advise given that if each patient is unique, so is the job and the company in which work.
We slept less than normal and on many occasions ill. It is very important to rest because it allows us to be in the best physical conditions and mentals to deal with the symptoms. Stories about our nocturnal periods of insomnia in which we do what we can: we use to «create» (writing, defining projects, doing crafts), we get bored, we become sad, we wake up our caretaker, the couple looking for someone with whom to talk and share these moments, or looking for someone who can massage a rigid and contracted muscles, or can help us lift and go to the bathroom, or countless things that one can do to do in those periods in the night that (for others) the calm prevails.
Apart from the movement and the march, who most of us have problems speaking when we vocalize badly, our tone is low and somewhat monotonous, and worse, we begin to have signs of the dreaded dysphagia, difficulties to properly swallow food. These difficulties can lead to aspiration, food entering the lungs and not going through the esophagus, and this can represent a very serious risk of infection and serious complications (pneumonia). This is why it is important to visit a specialist (speech therapist in this case) to see if there is dysphagia and, if it exists, to be able to receive adequate guidelines to eliminate or significantly reduce the risk of aspiration.
We have fears, anxiety, some depression, the day-to-day management of the family, the limitations will come, who will help us and take care when we need it. We bring it the best we can but it is impossible to be immune to it because we are all human and we are talking about effects that some of them put or may endanger our survival in the short term. Here we have everything; physical exercise, psychological or psychiatric help, self-help (relaxation techniques, anxiety and panic management), meditation, yoga, mindfulness, spiritual teacher tracking, alternative medicine, etc.
We usually take different medication, different in the type of medicine or different in the dose. It is very difficult at times to know the reasons why a patient takes A and another takes B, because it depends on the clinical history of each, the moment and age of the diagnosis, the neurologist (since it also happens that «each mastery» has its (whether shaking or stiffness, whether it is fast or not, etc.), the acceptance of medication by each, the «type» of Parkinson’s.
We see that specialists know more about this disease every day, but we continue to be subject (and go for long …) to «trial and error», to other times discussed medication adjustments (which almost always produce new side effects or collateral). In search of an acceptable quality of life by alleviating the effects of the new symptoms, the «monster» claws.
Here I feel fortunate that there are different medicines (and increasingly) to be able to «manage» reasonably both motor and non-motor symptoms and it is something that we always have to put in value and not forget. But I am aware of what a person with great experience in our health once told me and is that we are not terminal patients but we are and will be (possibly most of us) receiving chronic palliative care. Here, I reveal myself, and try to be patient and active as much as possible to help research and science so that everything goes a little faster in search of the ultimate goal (cure of disease).
We agree that when we have any health problem it is difficult, and sometimes very difficult, to conclude whether or not it is something derived or caused by Parkinson’s disease. And it is difficult for specialists, as imagining for general medicine …. I do not even want to think that one day I have to enter for any reason … .. having Parkinson … .. then I will be an active patient, expert … .. By instinct of survival, to make sure something as simple as continuing to take the right pills and to make sure there are no contraindications or interactions with the medication that hospital admission may require during the same.
And friends, we agree on the most important thing, that we have to be strong, always have a positive attitude, that we have to meet and talk and share much more often, that we have to help, that we never have to surrender, never. You know what that song of the Boss says …
In general we agree that the diagnosis changed our lives forever, becoming other people and that is where the opportunity or the solution is: we do not have to be brave and decide to do new things that one has in forever and it is not necessary because The disease has decided for us, it has changed our lives forcibly. As Francesc, my psychologist, said in an unforgettable writing published in an old post in this blog: welcome to yourself !!
And here is the management of both the complexity of the disease and the vital uncertainty: to live the moment, every second, doing what we like the most insofar as we can physically and mentally. And we do not have to explain or self-justify ourselves; try to do it and that’s it … it’s about turning a threat into an opportunity, and not giving it too much laps.
And the title, thunderstorm, would translate it as a storm, which is how I would define what is happening to me. If I think only of the motor and non-motor symptoms I had a year ago, my evolution is worrying. A year ago, I had no blockages or freezes of the march, I was not tired half as much now, I slept much better, I had no problem as to the voice or swallowing, I moved almost completely to normal time to turn around in bed, I was less sad and worried (logically).
Now, at this moment, I am much more medicated, my normal phases («on» phases) can be 6 or 7 hours a day, the rest being intermediate stages or «off» phases (I can not do anything, neither drive nor go to buy something, or practically to move from home). In part I think it is a product of the season (the heat consumes me and I regulate it badly), to have taken antibiotics, to stress and anxiety produced by the very situation that feeds that feeling.
In my last conversation with my neurologist he told me that his conclusion is that, today, I am «moderately fotut» (translated would be moderately bad) and that is very subjective given a value judgment, how is the word “moderate” for one may be more serious or less serious than others … .. But I stay with what «for now» I am so and I believe and I trust in that after the storm comes the calm.
And if not, it does not matter, I watch my 15-year-old son or my 6-year-old daughter for a moment, and I squeeze my fists to resume the march, to leave the blockade and continue forward. GO !!
It seems that tightening the cuffs helps because after a muscle contraction comes an expansion and that is the reason why I can move one of my two feet when I do that exercise. A little trick to close this post with joy, as always.
